►Please read the Faithful Soles - Business Advertisement Overview if you are not yet familiar with what is included with a FS business advertisement.

►Please note that your business can be either running or non-running related.

►Please note that being an advertiser on FS is not a guarantee of receiving business from your ad. It is up to the individual FS member or visitor to determine if they will or will not purchase specific products and/or services from any advertiser.

►The entire registration process takes approximately 5-10 minutes to complete, and must be submitted and approved to have a business advertised on FS.

►At the bottom of this form, you will click "Submit Form and Proceed to Invoice / Online Payment Screen" and be taken to the "Faithful Soles Business Advertisement Plan Selection and Payment Form" where you will choose the type of FS business advertisement ($250.00 for 1 year for a Geographic Advertisement Area 1, 2 or 3 advertisement, or $2,000.00 for 1 year for a Geographic Advertisement Area 4 advertisement, as described in the "Faithful Soles - Business Listing Overview"). When you have completed this step, you will receive a confirmation screen.

►FS reserves the right to make changes to the layouts of the advertising pages at any time at its discretion. Any down time due to changes made by FS to advertising pages will be added back to all existing advertisers expiration date (i.e., if the advertising section is down for 30 days and a business has an expiration date for their ad of November 30, 2007, then the new expiration date will be extended to December 30, 2007, etc.).

►Payment Method:

• If you choose to pay immediately via secure online payment using PayPal, choose the appropriate "Buy Now" button to complete the payment process. Paying via PayPal is recommended because it will have your advertisement online within 1 (one) business day, and you do not have to have an existing PayPal account to use this feature.

• If you choose to pay by regular check or money order, you will be emailed an invoice and your advertisement will be shown within 1 (one) business day of payment being received in full.

►Items below with an asterisk (*) are required.

*Business Name:   If you are a sole proprietor and do not have a business name, enter "Self". In these cases, your first and last name entered below will be used.

*If your business applies to Geographic Advertisement Area 1, 2 or 3, whereby you have a physical store location that customers go to, does your business have multiple locations that you would like to advertise?  Yes  No  (If you answered "Yes", please note that the first location is $250.00 for the year, and each additional location is $50.00 per year. Enter just one location in the space further below concerning physical address, and a Faithful Soles Account Services representative will contact you regarding billing and information for the other store locations.)

*Business Contact First and Last Name:  

*Contact or Business Email Address:   This may be the email of the contact person listed above (preferred) or a general email for the business. If you elect to pay by regular check or money order rather than PayPal, this is the email that must also be used for FS to email the invoice to requested on the next screen.

*Business Web Site Address (i.e. "http://www.mybusiness.com"): If your business does not have a web site, enter "None".

*Business Phone:   Please enter in a format such as "999-999-9999"

*Business Address: If you do not want to provide your business physical or mailing address, simply enter "Web Site" in the Physical Address section. You must have a web site address if you elect to not provide a business physical or mailing address in this section. You may use either a physical address or mailing address (if applicable), but not both on the listing.

*Main Business Category:   Click the down arrow for a business category list Running Shoes and Apparel Cycling and Apparel Swimming and Apparel Multi-Sport (Triathlons and Duathlons) and Apparel Health, Diet, Nutrition & Weight Loss Sports Medicine / Medical Other: Miscellaneous Products and Services

Other (if you selected "Other" from the dropdown list, please tell us your business' category): Businesses listed in the "Other" category will appear under "Miscellaneous Products & Services" for the respective Geographic Advertisement Area 1, 2 or 3. If your business is related to Geographic Advertisement Area 4, it will appear in the first available space of your choosing.

*Brief Business Description:  Up to 50 characters, including spaces. This verbiage will appear in your business advertisement on FS.

*Special Offers and/or Discounts Available to Faithful Soles members:

If there is a special offer and/or discount you would like to make available to Faithful Soles (FS) members, please type it in the space provided below, otherwise simply enter "None" in the space provided. If you do have a special offer and/or discount, it will be shown as a link next to your business advertisement that when clicked will open up a separate window, and only registered FS members will have the ability to access the special offer and/or discount information.
